- Complete your medical form. Click below, enter your name and email, and fill out the Diver Medical Questionnaire — it takes a few minutes and is sent securely to our physician.
- Schedule your telemedicine visit. After you submit, you'll get instructions to book your Doxy.me visit (doxy.me/apexprecisionhealth).
- Get your clearance. Our physician reviews your form, completes your visit, and issues your fitness-to-dive determination.
Fees (cash-pay): Basic Dive Physical $125 · Comprehensive $200 · spirometry add-on $75. Payment collected at/after your visit; itemized receipt provided. Veterans/active-duty 50% off.
